Community Based Assessment 

Community-Based Assessment    $60 per hour

Includes a job tryout in an employer’s business to assess factors such as the individual’s job interests, skills, abilities, work performance, quality, retention of job duties, work behaviors which may enhance and/or hinder employment, job accommodation needs, endurance/stamina, organization skills, environmental needs, and safety awareness.  

Development of Community-Based Assessment Site    $82 per hour

Includes job development efforts to locate/secure community based employment sites that will be appropriate to assess the needs of the individual as identified by the referring counselor and service provision with the consumer. Will include regular written updates at a schedule of at least one time monthly. 

Development of Site in the Area of the Individual’s Interest     $82 per hour

Includes job development efforts to locate/secure community based employment sites that will be appropriate to provide the individual with the opportunity to explore his or her interests and assess the needs of the individual as identified by the referring counselor, coordination of information sharing with individual, referring counselor, employer, and job coach, participation in team meetings. Will include regular written updates at a schedule of at least one time monthly. 

Job Shadow    $60 per hour

Facilitates an opportunity to observe different jobs and ask employers and employees questions about job duties, performance expectations, and job qualifications. A written report will be provided at the completion of Job Shadowing activities.

 Vocational Evaluation 

Comprehensive Vocational Assessment    $2600 Flat Rate per person

Includes collection of information through a comprehensive vocational interview and review of available documentation to identify work history, disability related issues, prior educational development, community/social factors affecting employment; administration of standardized Vocational Testing for academics, interests, aptitudes, and work temperaments; and combines a Community Based Assessment of 20 hours in a work setting consistent with the individual’s identified vocational goal to measure work and work adjustment behaviors. A written report will be provided at the conclusion of the evaluation. 

Vocational Assessment – Referral from RSC Defiance Office      

$650 Flat Rate per person

Includes collection of information through a comprehensive vocational interview and review of available documentation to identify work history, disability related issues, prior educational development, community/social factors affecting employment; administration of standardized Vocational Testing for academics, interests, aptitudes, and work temperaments; and completion of a Transferable Skills Analysis to determine possible vocational goal options for further consideration. A written report will be provided at the conclusion of the evaluation.

 Transferable Skills Analysis - Referral from RSC Defiance Office   $400 Flat Rate

Includes collection of information through a comprehensive vocational interview and review of available documentation to identify work history, disability related issues, prior educational development, community/social factors affecting employment; an analysis of the individuals previous work experience and educational development and use of computerized assessment tools to identify a Worker Trait Profile that determines what skills and abilities can transfer into other job options within the individual’s mental and physical capabilities. A written report will be provided at the conclusion of the evaluation.

 Transferable Skills Analysis       $500 Flat Rate

Includes collection of information through a comprehensive vocational interview and review of available documentation to identify work history, disability related issues, prior educational development, community/social factors affecting employment; an analysis of the individuals previous work experience and educational development and use of computerized assessment tools to identify a Worker Trait Profile that determines what skills and abilities can transfer into other job options within the individual’s mental and physical capabilities. A written report will be provided at the conclusion of the evaluation. 

Vocational Assessment Consultation, Collegiate Screenings, Clerical Testing    

$82 per hour

Includes home-site, on-the-job (Community Based Assessment), or worksite (when actually employed) observations to evaluate the individual’s capabilities of performing work tasks, determine need for environmental or procedural accommodations, and identify training needs. Can also be used to coordinate multiple Community Based Assessments for the purpose of evaluating skill-sets and abilities and specialized testing to identify specific abilities (such as academic abilities for the collegiate setting or clerical aptitude and skills/knowledge for employment). A written report will be provided at the conclusion of the evaluation.

 Employment Readiness Assessment          $750 Flat Rate per person

The assessment can be used with individuals who do not have a mental health diagnosis, or for those who do have a diagnosed mental disorder. It provides an in-depth look at the individual’s employment history and discussion of issues with past employment. This information is used to determine if the individual is ready to search for employment, or if other interventions are needed. Would be helpful prior to pursuing job placement. A written report will be provided at the conclusion of the evaluation.  **Note**: For either MENTAL HEALTH ASSESSMENT or EMPLOYMENT READINESS ASSESSMENT: If academic screening or LD assessment is requested in addition to these assessments, tools used to complete these evaluations will be charged as authorized by the RSC Psychological Fee Scale in addition to the fee for the individual assessment. TOOLS AVAILABLE for academic screening and LD assessment include: WAIS (Wechsler Adult Intelligence Scale), WIAT (Wechsler Individual Achievement Test), WRAT (Wide Range Achievement Test), and WMS (Wechsler Memory Scale). Tests needed vary depending on the individual’s needs. Other tests may be available upon request.

 Informational Interviews       $82 per hour

Provides for the opportunity to complete one or more employer interviews in the occupational area or job title of the individual’s preference or at the RSC Counselor’s instruction for the individual to ask questions about employment and educational qualifications, work experience, and job duties for that type of employment. Useful in making career decisions. A written report will be completed at the conclusion of all Informational Interviews.

 

Personal Adjustment 

Personal Adjustment         $60 per hour

Includes the provision of direct services to address the concerns and/or issues as identified by the individual and RSC Counselor. The scope of services provided will be flexible to allow for the needs of the individual to be met. A written report will be provided at the conclusion of the service, or at least monthly. 

 

Work Adjustment 

Work Adjustment One-to-One Instruction          $60 per hour

Includes instruction about expectations of work, including instruction and discussion on issues related to work, making career decisions, how work can affect benefits, how home life and the individual’s daily routine can affect work (and vice-versa), and transportation concerns. Curriculum is customized to meet individual needs. A written report will be provided at the conclusion of the service, or at least monthly. 

Work Adjustment Class (minimum of 4 participants)   $500 Flat Rate per person

Includes instruction about expectations of work, including instruction and discussion on issues related to work, making career decisions, how work can affect benefits, how home life and the individual’s daily routine can affect work (and vice-versa), and transportation concerns. Curriculum is customized to meet individual needs. A written report will be provided at the conclusion of the service, or at least monthly.

 

 Job Coaching 

Job Coaching    $55 per hour

Facilitates learning and/or social integration, offers supports as identified. Offers training in job performance, safety awareness and practices, supervisor and coworker interactions, and work behaviors. A written report will be provided at the conclusion of the service, or at least monthly. Job Development is at a separate fee.

  

Job Placement

 Job Placement         $82 per hour

Traditional Job Placement provides the individual the tools to complete an independent job search. Services include instruction regarding the job search process, including identifying and contacting employers, completing applications, developing resumes, and preparing for the interview process. May also include the use of a portfolio to organize potential and actual employer contacts, employment/educational qualifications, and materials to present in an interview, and scheduling for follow-up contacts. Will include regular written updates at a schedule of at least one time monthly.  

Performance-Based Services

For a maximum of six (6) months, or a total of 60 hours, then RSC will re-authorize, beginning at Phase I. The following phases do not include the services of Assessment and/or Job Coaching and/or formal JSST.

Job Development – Phase 1 (at referral)     

$2000 Flat Rate

Services include providing tools to individuals who have the ability to complete the job search independently, to assist the individual with the supports necessary to a job search. Supports may include application process, resume development, employer contacts.

Job Development – Phase 2 (at placement)    

$900 Flat Rate    

Job Development – Phase 3 (at closure)           

$900 Flat Rate     

 

Job Market Analysis     $82 per hour

Provides for a focused research of the labor market available to a specific career area or job title as directed by the RSC Counselor. A written report will be provided upon completion of the Labor Market Analysis.

 Diagnostic Job Development      $82 per hour

Job Development services specifically provided to create assessment sites, opportunities for career exploration and/or informational interviews, and to assess the labor market and potential for placement in a specific job title(s). Will include regular written updates at a schedule of at least one time monthly. 

Follow Up      $82 per hour

Service is provided following the placement of individual in employment. The Job Developer provides services to facilitate job retention. Will include regular written updates at a schedule of at least one time monthly.

 Job-Seeking Skills Training and/or Self-Directed Job Search       $82 per hour

One-to-One individualized instruction focusing on topics that the individual needs to enhance the job search. Will include development of a portfolio to organize job search and will include regular written updates at a schedule of at least one time monthly.

 Job-Seeking Skills Training and/or Self-Directed Job Search Class       

$500 Flat Rate per hour

One-to-One individualized instruction focusing on topics that the individual needs to enhance the job search. Will include development of a portfolio to organize job search and will include regular written updates at a schedule of at least one time monthly.

Employer-Employee Compatibility Analysis (EECA)    $60 per hour

Will assess similar areas as the CBA, but provided when a job is available at the completion of the job tryout. Will provide the individual the opportunity to determine if it were the job he or she would like to do. Also provides information regarding the individual’s skills and abilities to perform the job tasks, and determines if the work environment is “friendly” to the individual’s needs (i.e. social conditions, noise levels, heat/cold, etc.). Employer determines if the individual meets the job’s performance expectations. Will include a written report at the conclusion of the assessment. 

Development of EECA Site     $82 per hour

Includes job development efforts to locate/secure community based employment sites that will be appropriate to assess the needs of the individual as identified by the referring counselor and service provision with the consumer. Will include regular written updates at a schedule of at least one time monthly.

 Development of EECA Site in the Area of the Individual’s Interest      

$82 per hour

Includes job development efforts to locate/secure community based employment sites that will be appropriate to provide the individual with the opportunity to explore his or her interests and assess the needs of the individual as identified by the referring counselor, coordination of information sharing with individual, referring counselor, employer, and job coach, participation in team meetings. Will include regular written updates at a schedule of at least one time monthly.

  

Clinical Services 

Mental Health Assessment        $750 Flat Rate per person

This assessment includes assessment of mental health symptoms and how they impact employment based on past experiences and current symptomology. The assessment takes an in-depth look at the individual’s employment history and discussion of issues that may impact employment. This also includes a substance abuse assessment. A written report will be provided at the conclusion of the evaluation.

 

Counseling                  Follows the Psych Fee Schedule

Individual counseling is focused on problem resolution. Techniques used vary depending on the individual’s needs. Will include regular written updates at a schedule of at least one time monthly. Travel time will also be billed, but must be authorized at the rate of $43.00 per hour.

Other

 Financial Analysis     $82 per hour

May assist the individual with understanding benefits and the impact working can have on benefits. May assist the individual with development and monitoring of PASS or IWRE. Will include a written report at the completion of the service.

 Drug Testing      $50 Flat Rate per person

Fee is per test. Drug testing provides immediate results. This test is a standard 5-panel urinalysis, which tests for THC (Marijuana), PCP, Opiates, Cocaine, and Amphetamines. Testing is provided at the Defiance and Van Wert Offices. Will include a written report of test results.

 Background Check          $50 per hour PLUS cost of reports

 Transportation            $2.00 per mile and $35 per hour  

Fee is per mile AND per hour.

 Foreign Language or Other Interpreter Services      $82 per hour
